* I confirm having read and fully understood all the questions and answers in the FAQ

* I understand that acceptance of the Rescheduling & Restructuring (R&R) or my option to Retain the Original installment payments MAY:

affect my CCRIS records,
affect my future new financing/loan application,
require me to pay the accumulated one lump sum amount at once at the end of the financing period (bullet payment) and
increase the total cost of my financing and subject to not exceeding the Bank’s Sale Price.

*Mandatory to be ticked by customer before proceeding to apply for the program.
